WHĀNAU SATISFACTION SURVEY

Introduction

 Thank you for agreeing to participate in the Whānau Satisfaction Survey for Whānau Ora. The purpose of this survey is to understand what you and your whānau think of Whānau Ora services and how they can be improved.  

 You can fill out the survey individually or in a group/whānau setting. If you have any questions or need help filling out the form, please talk to your Whānau Ora provider.
